As for the Ultimate Challenge, you have to take an alternate route to get to it. See that western branch in the path you originally used to enter Gly'cov Village (Past)? That leads to the Ultimate Challenge. You need to press a few hidden switches here and there in order to complete it though, and need the Floatation Ring from Turtle Rock (beat the boss at the secret entrance, then open the previously sealed chest). Search in the Training Grounds and the Ocean (Present).

 

 

 

How do I get to the stairs from above ground? I saw nothing with or without the lens to show me where to go.

In the Training Grounds, go left one, down one, right one. Hookshot over to the tree and burn it to reveal the stairs.

I can no longer hookshot over to those stairs, since I opened them before I could go further inside the cave. And I can't jump or swim over, either...

There's another entrance roughly around the center of the Training Grounds. It's in plain sight, but it's blocked off by one of those switch blocks. If you have used the hookshot entrance before and followed the very small cavern to its end, it should be open now. If not, you probably need to download an update in which that probelm is fixed.

In level 7, I opened two doors going towards the bottom right side.  Can't find a key to open the third door in the room where my sub weapons are blocked

Can you provide a screenshot of what rooms you've been to? Level 7 is very open ended so it is hard to know which key you didn't get.

First pic - That is one of the issues I need to fix in the update. I thought I had fixed it, but alas, I did not.

thnks for the help with the map.  Trying to get some HCP's before level 8.  Can't find how to get to the secret room in b2 crater.  Used lens in every room.

I've actually made two video guides for level 1 and 2. I've considered doing other levels too, but just perhaps a bit later. If you watch the video at the 24:00 mark, you can see how to get that HCP.
