You could probably have an "item ring" item select menu that circles around Link or an item select bar displaying the next and the former item to the one currently equipped for L/R item switching with something similar to this. Way more complex obviously but it would have to locate Link and draw tiles above/around him.

FINALLY!!! Now my dreams of a "Where's Waldo?"-themed quest can become a reality!!
I'm kidding. This seems like a strange idea, but a bit of creative thinking and there's quite a few interesting ideas to pull off with this. For instance, I'm considering an effect sort of like the Invincibility graphics from the Sonic games.
