Super Mario 64
Doing the die for no reason glitch at Peach's Castle.
Trying to get that one Red Coin in Bowser in the Fire Sea floating over a hole in the the grating which leads to lava and you fall in it and Mario goes crazy hopping mad.
Dying while falling out of the Vanish Cap level and drowning when you come out of the hole.
Getting eaten by Bubba.

Contraption on the CPC - Click on the middle picture in this page - the plant to the left of the chap down the bottom. You lose a life if you step on that. You get "keep off the grass"-ed to death.

To name and MMORPG...
RuneScape has a Master Quest in which if you have at least one of the four diamonds (whiches are required to beat the quest) there is chance that an extreme powerful, almost invincible enemy appears to kill you. Near the end of the quest you need to carry the four diamonds to a piramyd in the desert. It's not advised to bring any battle items, because you need the inventory place for other things. So, you grab the four diamonds in the bank (it was insanely hard to get them, you barely survived), you run across the desert unarmed, you reach the piramid and the superenemy appears just before you place the first diamond...

Also, drowning in Ultima IX just after drinking an underwater breath potion while trying to loot the ghost ship of Bucaneer's Den.

In Baldur's Gate II, if you walked into the temple in the slums, sometimes a man would randomly pop in and throw an offering at you, which dealt one damage. No big deal, right? Wrong. The temple is the only place where you can revive your party members, and they come back with 1 HP. Imagine spending 2000 GP to revive your fighter, and then having some beggar kill them with a coin!

Final Fantasy X: Tidus not existing at all was a pretty crappy way to go.
