You will need to do stuff with screen state carry over to do this without scripts.

In the Zelda Classic folder there is a txt file called zstring. this file explains how to add "commands" to strings, allowing you to take items from link's inventory or vice versa, even switching to another string if link has a specific item.

In the Zelda Classic folder there is a txt file called zstring. this file explains how to add "commands" to strings, allowing you to take items from link's inventory or vice versa, even switching to another string if link has a specific item.
It's Sectipn 4.7c in ZQuest.txt
You first want:
\5\item_id\string
This causes a string to play a different string if Link has a specific item.
Play this before the start of the normal text in your string.
Then:
\16\item_id
This takes away that item.
Then:
\20\sound_id
This plays a special sound effect
Then:
\17\new_item_id
This gives Link a different item.
I think that you'll need 2.55 for Link to hold that item overhead, as 2.50.x does not have a default Pick Up Flags setting for items, Otherwise, this system works, and it's how this worked in 2.,10.

Screen state carryover is in the screen data menu. I think it's the third or fourth tab (don't have enough time to check rn). Basically, you choose a map and screen onto which you want to carry over the state (i.e. whether the item's been gotten, screen secrets activated, lock blocks unocked), and then choose which of those you don't want carried over. It's fairly intuitive, I think.
If you want the state carried over to more than one screen, you go to the screen you chose and do it again, listing the next screen you want carried over to. And so on.
