How do you make it so when you grab the triforce fragment, it teleports you to the beginning of the dungeon?

Try setting the Side Warp to Entrance/Exit, then select the DMap and screen for the side warp.

Also, you don't have to actually select a side for the Side Warp. Just leave all of the choices blank.
