-1) Head into the river and swim to the north dock. However, if you haven't cleared the cracked rock with a super bomb, and you don't have one, then take route 2, but go back to it later and blow it up. It's REALLY helpful. However, if you have blown it up, head two left, one up, and left again. Hookshot your way to the sign on the other side of the large holes and head up the ladder.
-2) Head west until you reach the forest - take the path route. When you enter the forest, head straight up out of the forest and at the overworld. There should be a river at the left side, with a cliff past that. Anyways, head up one and right one and then hookshot your way to the other side of the large holes, and head up the ladder.
Alright, up the ladder is the level three area. Up one should be the level itself. Left one of that should be the well, so head there first, as you can't proceed to level 3 without a key.
I like the key idea - it was in HoD, but it was too easy to access the keys. In this, you actually have to put forth some fighting skills and some brainwork to solve the puzzles.

That's... kinda vague. I don't think I recall going left after going through the lock block. Maybe you mean right? Well... this puts you in the big room (4 screens put together). ... I think I explained this before, but I'll say it again anyways. When you enter that room, head right, destroy all the enemies, and then push the two lower blocks on the right side of the screen down, as there is space. Pushing another block aside, you should be able to continue south by swimming through the water. That should get you somewhere.
Now... if you mean that you can't figure out what to do when you start off, well... even if you don't, I'll say it anyways for those who can't seem to figure it out. Take the upper passage on the left side of the screen. Destroy all the crates and you'll notice a symbol appear under one of the destroyed crates. Push the block onto it, and the door to the next room will open.

I got a rupee in the chest underwater.... Does that mean I have to start from the beginning of the game... Dang it, I don't feel like playing it over.
I don't know why it would change from a key to a rupee though. I am playing with 2.10 and I don't remember that bug. Please tell me I am missing something...

I think the correct way to make a "dive for item" is by using a "Special Item" Room rather than placing a normal item on the screen. I'm not sure though, because I never tried it myself.
It's a rather good conjecture though, because the chest contains a Rupee, which is the default "Special Item". So if you set the room's special item as a key, it should work.
If you fallow that method, the underwater chest should work properly.
Here it is in simplified form:
- Room Type: Special Item
- Special Item: Key
- Normal Item: -None-
- Flag: Dive for Item
