-speak with one of the girls in the main bath big room
-dive between the 3 swimming NPCs somewhere at the south
-Push the crate/pot at the north part of the big room
-Kill all bubbles on the east platform
I guess there's one at an other dive location
I think the girl doesn't count the first lock, that's why she says 4...

That's strange... it's a boss key door, always works for me and I never heard others comlaining about it... Maybe an enemy hit you exactly when you wanted to open it or such... Just try again.
EDIT: Ah, 50 pages...
This is in a room at the top of the engine room. There are 4 gold statues and two streams coming down. The door doesn't have a keyhole, and there are no enemies in the room. Are we talking about the same room?

Oh, we weren't talking about the same room! In that room that is a shutter at the north wall, of course no keys work on it!
Shutters can be opened in my many ways, for example by killing all enemies in the room. But what if you don't see enemies and you have evidence that they aren't just invisible rather completely missing from the floor? Well, then they are surely in the wall! Kill that 10 wall crawlers in the walls by shooting at all wall parts. Note that if you killed a wall crawler at a certain wall tile, there won't be one just next to it, so you can skip that spot, saving an arrow.

There's no lv4. There's IW L1. I guess you mean that you got to Mt.Dead End. You should leave it, it's a terrible place. But the exit is blocked. The only useful landmark is the dungeon in the northeast corner of the overworld. First slash the rubbles on the screen where you started to get an item that let's you continue. There are many caves, all lead south. At south, you can go east. At east, you can go north and can reach the northeast point. You need to use your freshly got item at the dungeon entrance to enter it.

"1 magic piece (wind)" can be found only at the very end, at the time of the last boss battles
"magic key" can be found using Medal of War. There's a place where objects arranged in a circle. Use medal on middle.
"boots" most ultimate secret, deep in the final dungeon.
"ultimate sword" is at northeast screen of main overworld (warp)
The Medal of War opens the final dungeon. Just be sure that the fire hits the magic seals (so you need to use the medal some steps away)
Megaman-ish place that needs keys? Only main dungeons, 8th subdungeon, Wacky World and final dungeon requires keys... Ther only Megaman-ish place that you can talk about must be the resting place of L4 sword, the Irrevelant (through gateway). It needs just a boss key.
The black hole others mentioned? I guess its the magic key's place. Use War Medal on circle. You already saw it at least twice.
