The Most Useful Items in Video Games.
The VS. Seeker (Pokemon FR/LG, Pokemon D/P) - Okay in Pokemon Ruby and Sapphire, you are unable to vs battled trainers again leaving you to only battle the Elite Four and the Battle Tower. Boring. Well Nintendo added the VS Seeker to FR/LG and D/P which made Pokemon games freakin awesome. The ability to VS previously battled trainers is the reason why I like to play Diamond. It's like pick up vs a trainer and trun off your DS.
Sword (Legend of Zelda Series) Without the sowrd there won't be a Link, if there's no Link than Ganon will rule the world.

Howabout... Transformation masks? From Majora's Mask. You can't do much without them.
Or better yet, the Ocarina of Time in Majora's Mask. Without it, the moon kills you.

Also, to beat ShadowTiger, the Gravity Gun. I've never played Half-Life 2(shame on me!) but it's awesome to turn anything not nailed down into a damage-causing projectile.
Oh, and grenades; when you can't shoot through a wall and the enemy is behind it, they become your best friends.

Boomerang. Doesn't use ammo, and you can usually collect stuff with it.
Also, to beat ShadowTiger, the Gravity Gun. I've never played Half-Life 2(shame on me!) but it's awesome to turn anything not nailed down into a damage-causing projectile.

When you get the gravity gun upgraded at the end of the game, you can pick up and throw people as well. Very fun.

2. Screw Attack (Metroid series) - Do I need to say more?
3. Hypermode (Metroid Prime 3) - Anything Phazon-based is bound to kick arse. (NOTE: Not a spoiler)
4. Soul of Hachi (Advance Wars: Dual Strike) - You're able to deploy ground units in any city during Super CO Power. What's not to like about that?!
5. Blue Spiky Shell (Mario Kart series) - homes in on the lead player (and, in Double Dash, makes a powerful explosion, as well). It owns so bad in Double Dash, if you're lucky, you can probably catch 2 or 3 other racers in the explosion!

Ah, ya beat me to it. I was just thinking of that. That's an item that makes a difference between everyone in the world dying and everyone in the world not dying.
Earthbound - PSI Teleport α - Seems like nothing more than a convenience at first, until you realise you can get to places you couldn't before.

Final Fantasy: MEGALIXER!! It's an Elixer, only 4 times better (or 5 times better in FF4 Advance) Lifesaver in the Lunar Ruins boss battles (especially the 50th floor boss. That was evil).
Oh, and TriMaster: You CAN refight trainers in Ruby and Sapphire! You could even in Gold and Silver! (I remember that there was something that was a sort of early Poketech in R/S that had a mode that let you see what trainers wanted to fight again. G/S had the telephone and the defeated trainers asked you if you wanted their numbers).

The double clawshots were pretty shweet.
