I've enabled the player to set items to either A or B, but I noticed that when I start the quest with only the sword, the sword appears in both button slots (until I collect an item, or sometimes it just fixes itself after a bit). Is there a setting I need to change?

Yeah I have had this issue too when you start the game with only one item. I'm not sure what the exact cause of it is, but it can happen with any item.

My theory is that it has something to do with how the inventory grid is laid out on the active subscreen, but I don't know for sure.

Does this also occur if you collect the sword as the first item in the game; or only if it's set via init data?

Yes. I am using 2.50.2, but I think I recall this appearing in of the previous versions as well, probably 2.50 RC3, I used to use that one a lot.
