Afaik the Overworld only re-randomizes enemies/enemy placements when you die, the terrain/map layout does not change. For dungeons everything re-randomizes when you re-enter the dungeon as well as when you die in one.

Key locations, like the areas containing the dungeons, are set, but most screens on the overworld are randomized. The general regions are always in the same place, I believe. For instance, the beach is always in the lower left, the desert in the lower right, mountains in the upper right, etc. However, any screen within these regions that isn't part of a dungeon entrance is randomized. I did a second run and never saw a screen that was identical to anything in my first run, except for those key locations.

My location observations are below, spoilered just in case. Keep in mind that this is just what I've figured based on what I've seen, and can possibly be wrong.
