Assigning items to button?

Yes, but:
If you want to be able to assign the sword to either the A or B button, you will also need to add the sword
to your subscreen as a usable item, and you will have to set it's position and cursor selection values to
work with the existing button items.

To do this correctly you have to understand how this works, every item is given a number, and these numbers communicate with each other when making them selectable. Not only do you have to modify the numbers of the sword item, you also need to modify the surrounding items so that they also move to your new desired item.
For example:
Let's say if
Boomerang is 3
Bow and Arrow is 2
Magic wand is 4
You want to replace the magic wand with the sword. You simply give the sword all the numerical value the wand currently has.
Now let's say if you want to put the magic wand in a new location and give it number 16, and it would be located above the arrow.
As it stands:
Boomerang is 3
Bow and arrow is 2
Sword is 4
Magic wand is now 16
In numerical value, you will want to edit the arrow so the up direction is 16, and edit the magic wand so the down value is 2.
Say if you don't want to replace the magic wand with your sword, and simply just want to make the sword appear above the bow and arrow. Let's say the number you choose again is 16. The same conditions apply. The up value for the bow and arrow would have to be 16, and the down value for the sword will have to be 2.
You will have to modify every directional value for the new item you want to place, and modify the numerical values for every item surrounding it to make it work right.

If there is a way to prevent two specific items from being assigned at the same time I am not aware of it.

To my knowledge with that quest rule turned on, you can set any item to A or B.

ZC doesn't allow for the same item to be assigned to both A and B buttons at the same time. If you do attempt that, ZC will switch the items in the slots around (A with B and vice versa) like in the GB games.

I understand that, but if I understood ABH15 correctly, he didn't want link to able equip the hammer to
the second button if the sword was out and vice-versa.
Thus the hammer and sword are what I was speaking of when I said I don't know how to prevent
two specific items from being assigned at the same time (to different buttons, not the same button).

Would nulling the a and b input if link is using the hammer or sword work for you?
How would this be done exactly? I'm pretty sure it needs to be scripted so can anyone write that for me? If so, thanks! If not, then oh well I can make do without it.
